Accessing Your Health Information
At Zimran Orthopedic Medical Centre, you have the right to access your own personal health information in accordance with applicable health privacy laws.

You can fill out the form directly on your computer, then print and sign it before mailing, delivering, or faxing it to Zimran Orthopedic Medical Centre. (Please note that faxing is less secure than mailing or hand delivery.) For your privacy, an electronic copy of the completed form cannot be saved, so be sure to print a copy for your records. We will process your request as quickly as possible, though we are allowed up to 30 days to respond.

Death Certificate
We issue death certificates for patient who died under our care during critical circumstances. In the event of a patient’s passing, Zimran Orthopedic Medical Centre will complete a Medical Certificate of Death, which will be given to the family of the deceased. To obtain a death certificate, please contact your local Service Centre or visit their official website.
Corrections to Your Health Record
If you believe that any part of your personal health record is inaccurate or incomplete, you may submit a completed Request for Correction to personal health Record form to our Record Department. We will review your request and respond within 30 days, as required by health privacy regulations.

Your personal health information is used and disclosed to treat and care for you and to:
